What are advance directives?

These legal instructions detailing who can make decisions about your health care if you're unable to communicate your wishes. Advance directives are made of two documents: a durable health care power of attorney and a living will.

What is a durable health care power of attorney?
It makes your wishes about future health care decisions clear in the event you're not able to yourself.

What is a living will?
It makes clear whether or not you want your doctors to take measures to keep you alive if you're not able to communicate your wishes.
