Pay online with a MyChart account. See your bill for activation code. We also offer payment by phone or mail.

An overview of the bill's contents, including patient information. For an itemized list of charges, flip the bill over.

4. Perforated Check Stub

Simple instructions for paying by mail. Include this stub in the envelope with your check.
