Access Multiple Accounts

You can request access to see another person's medical information. This allows you to communicate with that person’s doctor and health care team on their behalf.

Requesting Access to a Family Member’s MyChart Account

Return the completed form to your primary care physician.
You can also return the form to the Health Information Management department at Lexington Medical Center, located at 2720 Sunset Blvd, West Columbia, SC 29169.
