Pre-Registration Phone Call

You will be contacted by the admissions department (for services at Lexington Medical Center) or an outpatient registrar (for services at LMC Irmo or LMC Lexington) to gather the information to pre-register you for your stay.

Please prepare by having:

  • Social Security Number
  • Insurance Information
  • Insurance Authorization Number

Pre-Admission Interview

Before your stay, our interview and assessment staff will schedule a convenient appointment to gather key health information and to schedule any required pre-operative tests ordered by your physician. Patients are seen by appointment. Walk-ins are worked in as the schedule permits.
  • Expect the interview to last about 90 minutes.
  • Bring all regularly taken medicines and herbal supplements.
  • Minors (under the age of 18) must be accompanied by a parent or legal guardian.

Insurance Authorization Number

Some insurance providers require authorization for certain hospital services. Upon approval for a procedure, the insurance company will provide you with an authorization number. You are required to provide that number during pre-registration and again during your pre-admission interview.

If you do not have an authorization number, your surgery may be delayed or rescheduled. If you do not secure authorization from your insurance provider, any services performed may become your financial responsibility.
