Your physician's office will schedule your surgery with the hospital.
If you are scheduled for Outpatient Surgery you will not be staying overnight in the hospital.
If you are scheduled in our A. M. Admission program, you will not be in the hospital before your surgery, but will require hospitalization after your surgery.
Pre-registration
Before your surgery, you may receive a telephone call from the admissions department to obtain preregistration information.
Your upcoming procedure requires approval and an authorization number from your insurance company. Please obtain that number prior to your Interview & Assessment appointment and bring the number with you to your appointment. If you do not have the authorization number, your service may be delayed or rescheduled until we can obtain the number. Please be aware that unauthorized services can become your responsibility.

If you have not received a call at least 5 days before the date of your surgery, please call this number: 791-2314 to schedule a time.

Please bring your medicines or a list of your medicines, herbal products and over the counter medicines you take daily. Please include dose and number of times taken each day.

Bring the Outpatient & A. M. Admission booklet and other papers or instructions your doctor may have given you.

Minors (anyone less than 19 years old) must be accompanied by a parent or legal guardian.
